1ab9884cb2 Update copyright notices 
This also:

1. updates copyright to 2011-2019,
2. adds missing headers,
3. harmonizes to "Contributors to the Eclipse Foundation"
   (replacing a "Copyright Red Hat, Inc." by that is correct).
